How does the continuum of care work?
At Pathlight, we offer every level of care — from inpatient to outpatient and virtual treatment. During our comprehensive assessment process, Master’s-level clinicians get to know your patients, understand their needs and place them where it’s clinically best, with a plan designed to help them move forward at an appropriate pace.
As your patients grow, treatment adapts. New structure. New skills. The right challenges at the right time help patients build confidence to move forward. The same clinical model, approach and interventions guide each stage of care, so transitions feel seamless, and every step forward feels possible. Because the right care from the start leads to the right outcomes in the end.
